Named GP

From 1st April 2015, under the terms of the GP contract, all patients are required to have a named accountable GP. All patients in this Practice have been allocated a named GP. Your named accountable GP will be the named GP you are registered with. This GP has responsibility for the coordination of services required under the contract and to ensure they are delivered to each of their patients, where required, based on the GP's clinical judgement.

If you do not know the name of your accountable GP and wish to do so, please ask the Receptionist.

If you have a preference as to who your named accountable GP is, the Practice will make reasonable efforts to accommodate this request.

Although you have been allocated a named GP, this does not prevent you from seeing any of the GPs at the Practice.

Patients initially registered with Dr Lesley Foskett

If you were initially registered with Dr Lesley Foskett, as Dr Foskett has now retired, your named accountable GP will be Dr Yadavakrishnan Pasupathi.
