New Patient Registrationregister_now

Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your address. In this respect please bring along proof of address such as a recent utility bill. Our Practice covers the following areas:

  • Heanor
  • Marlpool
  • Shipley - up to 'The Field'
  • Loscoe
  • Langley Mill
  • Codnor - up to Steam Mill Lane
  • Smalley - up to Bell Lane

We also have some patients registered with the Practice who live in the Eastwood area but we do not accept newly registering patients for this area. In addition, exisiting patients of ours moving into this area, will be required to change to a more local doctor.

Extended Practice boundary for existing patients - (Shipley-up to The Copse) and (Smalley-up to Morley Crossroads) - existing patients of ours who move into these areas, may remain registered with the Practice.

You will need to complete a registration form (GMS1) and a health questionnaire, which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team. If you are taking medication, please bring this with you to your health check appointment. It is compulsory that all new patients on medication, attend for a health check.

Medical treatment is available from the date of registration.  Please contact reception for further information.

Preference of Practitioner

While registered with the Practice, we encourage patients to see any of the doctors and clinical staff. However, you may, if you wish, express a preference to receive services from a particular doctor. Once a preference has been indicated, we shall endeavour to comply with the request. However, there may be times when this is not possible, eg. doctor unavailabiliy, or the services requested are not provided by your preferred doctor. Please also not that should you wish to see your preferred Doctor, then this may result in a longer waiting time for an appointment.
