Patient Participation Group
To enable patients to become involved and give their views about the services provided by the Practice, a patient group was formed in 2011. The group meet at the Practice every 6-8 weeks. If you are interested in joining, please read the information above 'what is a patient group' and 'what can patient groups do' and complete the following form. Please return the form to the reception department.
If you would like to be involved but are unable to attend meetings, we are also setting up a virtual patient representation group. We are compiling a list of email addresses so that we can contact you from time to time, to seek your views and opinions on the things that affect you. We will try and keep our surveys short and to the point, so it shouldn’t take too much of your time. In order to get a truly representative sample we need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.
If you are happy for us to contact you occasionally by email, please click the link below to open the sign-up form and complete all the fields.
